What Does Rhinoplasty (Nose Job) Mean?

Rhinoplasty (nose job), derived from the Latin word Rhino, which means nose, is the name of aesthetic nose surgery in the aesthetic/plastic/reconstructive surgery literature and its widespread use worldwide.

Its primary purpose is based on reformatting the image of the nose. Especially from the patient’s point of view, rhinoplasty is primarily related to the nose’s appearance. Still, from the surgeon’s point of view, aesthetic regulation is the most important part of rhinoplasty, but it is not the only one.

What Is Rhinoplasty?

Rhinoplasty (nose job) is a surgical procedure that can be performed with open or closed techniques to eliminate visual complaints about the back of the nose, the tip of the nose, and the length and size of the nose. The nose is reshaped to match the facial features, considering the patient’s expectations.

Having a smooth and harmonious nose with the anatomical structure of the face allows us to achieve a facial expression close to the measure of beauty known as the golden ratio, so the nose is one of the essential criteria in the aesthetic appearance of the face.

The nose is a highly complex structure consisting of bones, cartilage and soft tissues that work our sense of smell, breathing, move, but are also affected by heat. Rhinoplasty involves applying aesthetic and functional surgery, which is performed considering all these parameters, the aesthetic and affective dimensions of which are equally important.

How Is Rhinoplasty Performed?

Rhinoplasty is performed with two basic approaches: closed rhinoplasty and open rhinoplasty. Whether the operation will be performed by an open or closed approach and what techniques will be used are explicitly planned for each rhinoplasty patient.

Surgical Techniques Used in Rhinoplasty Operations

Open rhinoplasty and closed rhinoplasty are the two main techniques used in rhinoplasty surgeries.

Considering the anatomy of the nose, skin structure and other aesthetic parameters, the appropriate one for open or closed rhinoplasty operations is determined by the surgeon in such a way as to achieve the result closest to the patient’s request.

What Is Open Rhinoplasty? How Is It Performed?

Seventy per cent of rhinoplasty surgeries performed worldwide are performed with an open technique. This is because closed rhinoplasty is not the proper method for every nose. Open rhinoplasty expands the possibilities of the operation and allows you to get some soft tissue in thick-skinned noses, as it will enable you to intervene in seeing the anatomy of the nose.

Especially in thick-skinned nose surgeries, open rhinoplasty is preferred for large and aquiline noses. However, the surgery method is an open topic for discussion among doctors.

Open nose surgery, which was the only valid method in aesthetic nose surgery until recently, is the classic rhinoplasty method. It is performed under general anaesthesia by removing the skin with incisions opened under the nose.

Open rhinoplasty surgery reshapes the nasal bone. Cartilage tissues eliminate functional problems that lead to respiratory issues such as deviation or nasal flesh, which we call intra-nasal bone and cartilage curvature.

Open rhinoplasty surgeries are performed to eliminate a wide range of nose tip aesthetics, nasal hump aesthetics, nose reduction, nose shortening, nasal tip elevation, and nose curvature.

The main thing is to choose the proper technique for the right nose and perform the operation in the best possible way.

Advantages of Open Rhinoplasty:

Open rhinoplasty allows restoring the anatomy of the nose just like lego. The available method is our choice when we want to reconstruct bone, cartilage, and connective tissues.

According to the closed rhinoplasty technique, our dominance area is more comfortable than the open rhinoplasty method. Since it is easy to work in open nose surgery, we can give the nose the shape we want and make a perfect working nose.

After open nose surgery, the scar is too weak to be seen with the naked eye. This small incision under the nose is noticeable only if you look closely at it. In this term, it is not so different from the closed method.

Disadvantages of Open Rhinoplasty:

According to the closed method, tissue trauma is more—edema, bruises, and swelling after open rhinoplasty occurs more often than in the closed form. Swelling after nose surgery is a very typical result. Depending on the skin structure and the content of the operation performed, its severity varies in each case.

What Is Closed Rhinoplasty? How Is It Performed?

The technique of closed rhinoplasty is performed without opening the incision. It is a technique of aesthetic nose surgery performed in a secure method thanks to small incisions opened through the nostrils on the noses that can be corrected with minor interventions.

It is possible to correct the back of the nose with closed nose aesthetics on noses that do not require intervention at the tip of the nose, which look beautiful when viewed from the front, but look high or hump from the profile.

Advantages of Closed Rhinoplasty:

Bruising and swelling after closed rhinoplasty becomes less. The healing process is shorter than open rhinoplasty.

Disadvantages of Closed Rhinoplasty:

Closed nose surgery is not a suitable method for everyone; open nose surgery is a valid surgical technique in cases where the tip of the nose needs to be reconstructed.

Technically, what we can do with closed rhinoplasty surgery, which we perform by removing a piece of the nasal bone, is limited.

Methods of Approach to Rhinoplasty

We can mention four techniques that we use in rhinoplasty surgeries, both open and closed methods, to shape the nasal bone and soft tissues:

Ultrasonic Rhinoplasty (Piezo Technique) 

Both open and closed rhinoplasty of the nose is also used with the Piezo method. This technique, which we use to shape the nasal bone, reduces surgical trauma in rhinoplasty.

The piezo instrument used in ultrasonic rhinoplasty does not damage soft tissues, vessels and nerves while shaping the nasal bones during surgery.

With piezo nose aesthetics, we can achieve prismatic folds at the tip of the nose to reflect light refractions in thin-skinned noses. We can thin the soft tissue to a certain extent thanks to nose tip surgery on thick-skinned noses.

Conventional Rhinoplasty (Classical Rhinoplasty)

Conventional aesthetic nose surgery, which we call classical rhinoplasty, is a traditional method used in open and closed rhinoplasty.

Rhinoplasty with Burr

It is a method of forming the nasal bones without breaking and removing their excess using low-frequency diamond and carbide burr tips during surgery. It works very well in selected cases. It is impossible to apply it to every nose.

Combined Rhinoplasty

Combined rhinoplasty is a term we use to refer to two different procedures in rhinoplasty surgeries.

The surgical approach, which we apply by combining ultrasonic, burr and classical methods, is combined rhinoplasty. Both piezo and burr are devices that can be used simultaneously. The combination of experience and technology allows one to plan personalised approaches rather than operating each patient with the same technique. Thus, we can set up procedures that will provide maximum benefit.

Combined rhinoplasty is also used to refer to the situation where rhinoplasty, filler, botulinum toxin injection, rope suspension and other similar facial aesthetic components are performed in the same surgical session.

Types of Rhinoplasty (Nose Job)

Types of rhinoplasty surgery that vary according to the structure of the nose, the area to be treated, the number of operations and the method of operation.

Septoplasty (Deviation Surgery)

Septoplasty is a nose surgery performed to eliminate disorders in the nose that we call deviation. In deviation surgeries, the external appearance of the nose is not interfered with.

The septum is the name of the structure located in the inner part of the nose and symmetrically dividing the nose in half, consisting partly of bone and partly of cartilage. The curvature of this structure due to congenital or acquired traumas is called septum deviation, and the operation to correct this curvature is also called septoplasty.

There are no changes in appearance after deviation/septoplasty. It is a surgical intervention performed to improve the function altogether. There are no bruises and swelling.

Septorhinoplasty (Rhinoplasty)

In septorhinoplasty, we aim to ensure that the nose, which is corrected functionally and formally, is both aesthetically suitable for the face and functionally works optimally.

Tip Plasty

Tip Plasty is just a nasal tip surgery. It covers operations aimed at the tip of the nose to eliminate problems such as droopy nasal tip and curvature of the nasal tip.

Tip Plasty has recently become widely used in patients who want to get rid of defects at the tip of the nose but are satisfied with the back of the nose.

Since the tip plasty requires the remodelling of the cartilaginous structure that makes up the tip of the nose, the open tip, the last method, is usually used.  The droopy nasal tip is removed, the curvatures are corrected, the tip of the nose is reduced if it is large, the asymmetry problems are eliminated, and the distance between the lip and the tip of the nose is brought closer to the aesthetic ratio.

Functional Endoscopic Sinus Surgery

Functional Endoscopic Sinus Surgery (FESS) is another type of nose surgery performed using small and sensitive surgical instruments while viewing images obtained with endoscopic systems from a monitor in the sinuses, which are on our face and are severe anatomical points.

Revision Rhinoplasty

Revision rhinoplasty is the second nose surgery performed in patients who have had rhinoplasty before; in cases where the nose surgery needs to be repeated because they do not like the aesthetic appearance and because they are uncomfortable with the functional deficiencies of the nose, or depending on whether both complaints are related at the same time.

Non-Surgical Rhinoplasty

Non-surgical rhinoplasty, also called injection rhinoplasty, is not a surgical procedure. The performed filling application serves to correct the deformities of the nose. The nasal filling provides a limited and temporary benefit. Currently, it is a method that is increasingly preferred by individuals who are afraid of surgery. It is used to eliminate problems located on the middle line of the nose. It provides a significant improvement in the profile image.

Ethnic Rhinoplasty

The shape of the nose and the structure of the face vary depending on the climate and geographical conditions. An ethnic rhinoplasty is an aesthetic approach to making the small noses of Far Eastern or African-American individuals with a weak cartilaginous structure look prominent and more significant by their facial features. 

With the operation of ethnic rhinoplasty, the wide nostrils are reduced in size. Cartilage grafts support the shallow, nasal bridge and nasal back. The droopy nasal tip is brought to an optimum level. While all this is being done, it is observed to obtain an image compatible with the chin and forehead area.

Since poor cartilage support is the main problem, one’s cartilage tissue is usually used to obtain strong cartilage support. Rarely cadaveric cartilage can also be used. It is possible to achieve ridiculous results in ethnic rhinoplasty surgeries that require profound experience and sensitivity.

Types of Rhinoplasty According to the Problem of the Nose

1) Crooked Nose Rhinoplasty

A crooked nose is a deformed nose that slides to the right or left when viewed from the opposite side or forms s-shaped curvatures that contain both of them together.

Fixing it is a pretty challenging and challenging task. We strive to bring the nose to the middle line with delicate and meticulous work, creating the desired fractures and using several grafts.

In this type of nose, our preferred method is septorhinoplasty surgery.

2) Aquiline Nose Rhinoplasty

The nasal hump deforms the profile image. Depending on the degree of the height of the hump, the severity of the aesthetic effect caused by this deformity varies. The posture of the eyes, the tip of the chin and the lips are overshadowed by a large protrusion, and they look different than they are.

The nasal hump, which usually appears later in adolescence, can gradually grow, and among the defects of the nose, it is the most disturbing one.

We can correct the nasal hump with open rhinoplasty or closed rhinoplasty surgery. The profile image changes entirely after the nose surgery are performed with the person’s appropriate technique.

Thanks to removing the hump on the back of the nose and repositioning the nose tip, we obtain an aesthetically perfect line from the eyebrow to the chin tip.

3) Broken Nose Surgery

 These are the types of noses in which the functional and formal abilities of the nose are weakened or lost as a result of traumas such as physical intervention, impact, and traffic accidents.

4) Nostril Problems

Correction of problems such as the fact that the wings of the nose are too broad notches seen on the sides of the nostril when viewed from the opposite side and the nostrils are visible, very pronounced nostril asymmetries due to the shift of the cartilage of the tip of the nose, is evaluated within the scope of nostril aesthetic problems.

5) The Nose That Can’t Breathe 

Swelling of the concha inside the nose due to curvature of the nasal bones and cartilage, polyps, which is a pathological finding, and valve problems that occur with the appearance of a squeezed nose are the main problems that lead to difficulty breathing through the nose.

With the correct preoperative process and planning, these problems are eliminated during the rhinoplasty process.

6) Droopy Nasal Tip (Tip Plasty)

 When viewed from the side, the height from the base of the nose to the tip of the nose should make a certain angle with the lips. The narrowing of this angle becomes noticeable with the convergence of the end of the nose to the lip. It is easier to observe that this angle narrows in patients with long noses.

With tip plastic surgery, it is possible to significantly correct both visual and functional recovery by placing the tip of the droopy nasal tip into the anatomical position where it should be.

7) Stretched Nose

It is a condition characterised by the fact that the nose rises to form a hump in the cartilage and bone region and takes a shape that will make the tip of the nose look long and low.

In short, it can be described as an image of a letter that is large in its envelope. There is a hook-looking nasal bridge in most patients when viewed from the side.

By detecting the problem correctly, we can get the image we want by making the cartilage and bone structure of the nose suitable for the skin by the anatomy. In other words, we can say that stretched nose surgery is a way to adapt the dress to the body.

8) Wide Nose

Suppose the nose is more comprehensive than it should be. In that case, a deviation in the golden ratio may occur, which is the decisive and universal criterion for finding the face aesthetically attractive.

For example, if the posture of the wings of the nose exceeds the distance between the two eyes, which is common in ethnic noses and when the height of the nose is short, or in traumatic noses, the wings of the nose spread thicken. This, in turn, leads to relatively large nostrils, and the golden ratio is impaired.

In the same way, the problem is detected correctly, and with the proper techniques and aesthetic approaches, we can achieve the image we want on the wings of the nose. Sometimes the width of the nose can only occur due to the expansion of the nasal bones; it is also possible to eliminate this condition with sidewall aesthetics.

9) Thin-Skin Rhinoplasty  

Sometimes the skin of the nose becomes so thin that the underlying bone and cartilage skeleton is completely exposed. Therefore, working carefully in rhinoplasty operations with light skin is essential. It should be carried out with maximum accuracy and with the correct methods of grafting, not forgetting that minor defects will look very large.

10) Thick-Skin Rhinoplasty

Since the patient cannot change the skin structure s/he has, s/he must first understand the current situation, at which point the physician’s responsibility increases. The doctor and the patient must understand each other at a realistic expectation point. Otherwise, there may be disappointments.

It is quite possible to have a thick-skinned but aesthetically extremely beautiful-looking nose.

In patients with thick skin, it is possible to thin the skin as much as possible, reduce the subcutaneous tissue to physiological limits, and achieve pronounced type definition using the correct cartilage grafting techniques.
